NOTE: Choosing an insurance plan from this list is not sufficient. Since
there are many versions of each insurance plan, please verify with the
insurance company that we participate in the specific plan you are considering.
We can only submit to insurance plans with which we participate. Your
copay is expected at the time of service. We do not submit to secondary
insurance.
If we do not participate with your insurance, payment is expected upon time of visit and a receipt will be given to you for reimbursement purposes.
There is an extra charge for Sunday and holiday visits, and there is a $25 charge for missed appointments. Please call us if you need to cancel an appointment or if you know you will be late!
