We wish to welcome all our new and current patients and
encourage you to utilize our Maple Avenue Pediatrics Web Site. If you are
visiting our practice for the first time, use these helpful links to review the
HIPAA
Privacy Policy. You can then print an Acknowledgement
of Receipt of Privacy Policy, New
Patient Form and Child’s
Health History form, complete them at your leisure and bring them with you
for your first visit. This will help to save time before your visit with the
doctor.
We have also received a “new” vaccine in the office. We are now giving Prevnar-13 to replace
Prevnar-7. This vaccine contains 13
subtypes of Streptococcus pneumonia. It
will provide better protection against meningitis, blood infections, pneumonia
and ear infections. We will be giving
one dose of this vaccine to all children under the age of 5 who completed their
Prevnar-7 series.
We have also begun to offer Gardasil to boys ages 12 and
up. This vaccine has already been given
for several years to girls. The vaccine
provides protection against the 4 subtypes of Human Papillomavirus with the
largest disease burden. It is a 3 dose
series in which the second dose is given 2 months after the first dose and the
third dose is given six months after the first one. In boys the vaccine protects against genital,
oral, and anal warts and cancers. It
will also protect against the transmission of HPV to females where it can cause
cervical cancer.

and summer physical appointments now!

· * *Updated 6/16/10** Currently, physicals may be scheduled up to September 19th!
- * *Update 5/4/10** McNeil Product Recall!! All Tylenol infant and children’s
preparations have been recalled including Tylenol Plus suspensions. All Motrin infant drops and children’s
suspensions as well as Zyrtec suspension in bottles have been recalled
too. Benadryl liquid in bottles
have also been recalled. They are
being recalled because some products do not meet quality control
standards. Some of the products included in the recall may contain a higher
concentration of active ingredient than is specified; others may contain
inactive ingredients that may not meet internal testing requirements; and
others may contain tiny particles.
You may use the generic forms of ibuprofen
and acetaminophen. Advil may also
be used. You may also use generic
forms of cetirizine (Zyrtec) and diphenhydramine. Please call us if you
have any questions!

- Please check with your insurance company prior to all physicals
whether you have a co-pay for well visits.
Also please check prior to all visits which laboratory your
insurance uses.
- The NJSIAA has issued a new Athletic Pre-Participation Physical Exam Form that is required for participation in any Junior High and High School sports programs. If your child is age 10 and older, is scheduled for a physical exam, and is going to participate in any sports in the upcoming year, you must click on the link above, print out the form, and fill out your sections of the form PRIOR to coming to the office. These forms are quite thorough and need to be reviewed by the doctor. Also, please speak to your school administrators about whether a vision screen is required for sports clearance. There will be a $10 charge for each of these forms. If you do not request a sports form at the time of your child’s physical, there will be a $20 fee to have one done at a later time.
